Session Initiation Protocol (SIP) is the signaling standard that sets up, modifies, and tears down real-time communication sessions over IP networks. Defined by the IETF in RFC 3261, it powers most enterprise VoIP, video conferencing, and instant-messaging traffic, acting as the dial tone of the internet for modern unified communications.

SIP borrows heavily from web protocols. Its text-based syntax mirrors HTTP, and its addressing scheme — sip:user@domain — looks like an email address, which makes it easier for developers to read traces and debug call flows.

How it works

SIP works by exchanging short text messages between endpoints called user agents, routed through proxy servers that find the right destination. The protocol handles five core functions. User location finds where a person is reachable. User availability checks whether they can take the call.

User capability negotiation confirms both sides speak compatible codecs. Session setup exchanges the parameters needed to open a media stream. Session management lets either party transfer, hold, or end the call mid-flight.

Behind the scenes, SIP relies on a handful of supporting components — registrars store user locations, proxies forward requests, redirect servers point callers elsewhere, and back-to-back user agents bridge two legs of a call. Media itself travels separately over the Real-time Transport Protocol, not over SIP.

SIP traffic typically runs on UDP or TCP port 5060, with TLS-encrypted SIPS on port 5061. The IETF maintains the full method and response-code registry through IANA, which is the authoritative reference for any production deployment.

  • Telehealth and remote consults. US providers route patient video calls through SIP-signaled platforms that meet HIPAA logging requirements, often with offshore agents handling intake.
