Clinical Outsourcing

Clinical outsourcing is the practice of paying an outside organisation to run clinical work a sponsor or provider would otherwise staff itself. It covers trials, coding, documentation, and patient support, and the regulatory duty stays with whoever owns the study.

That last point catches buyers out — you can transfer the work and the people, but the inspector still knocks on your door when a record is missing.

The category spans two very different worlds. One is research, where a sponsor buys trial capacity. The other is care delivery, where a hospital or clinic buys documentation and administrative help.

Both share a hard boundary — clinical judgement about a named patient stays with the licensed clinician responsible for that patient, wherever the work is performed.

How it works

A sponsor or provider defines the scope, then contracts a specialist to deliver it under written obligations. The contract names each duty being assumed, the standards that apply, and the audit rights the buyer keeps over the work.

In research, the transfer is formal. Under 21 CFR 312.52, published on the eCFR, a sponsor may transfer any or all of its obligations to a contract research organisation, but the transfer must be described in writing and anything left out is deemed not transferred.

In care delivery the mechanism is looser but the discipline is the same. Credentialing files, licence verification, and access logs carry the weight that a written obligation transfer carries in research.

Access provisioning is the usual delay. Granting an offshore team into a clinical system takes longer than hiring the team, because security review and licence checks run in series rather than in parallel.

Price is rarely the deciding factor — buyers pick on audit history, turnover, and whether the provider can produce a clean file when a regulator asks for one.

A mid sized biotech. It ran a phase two study through a contract research organisation in 2024, keeping protocol authorship and data ownership while buying site management and monitoring.

A US hospital group. Overnight clinical documentation moved to a Manila team working live in the electronic record. Physicians kept sign off, and the provider was measured on turnaround and query rate.

A specialty clinic. Coding and charge capture went offshore. Audit sampling stayed with an in house coder who reviewed a fixed percentage of charts each month.

A pharmaceutical sponsor. Pharmacovigilance case processing was contracted out with a written obligation transfer naming each duty, so nothing sat in the gap between the two organisations.
